LIABILITY WAIVER and RELEASE FORM - POLE DANCING & ANY OTHER FITNESS CLASSES 

 

 

Please review these policies carefully. In consideration for being permitted to participate in the Pole Dance class and any other activities conducted by Zero Gravity Pole. You will be required to read, sign and return this form before being able to commence a class with Zero Gravity Pole and any of its Instructors.

 

RELEASE AND WAIVER OF LIABILITY

 

Pole dancing and related fitness classes are a dangerous activity: I understand, acknowledge and agree that pole dancing & other fitness classes are a dangerous activity and as such, has inherent dangers and risks, including risk of injury or death.

 

I further acknowledge and agree that due to the nature of pole dancing and the other fitness activities offered by Zero Gravity Pole, it would be unreasonable for Zero Gravity Pole to be any way responsible for any injury of any kind or death, and I hereby, to the full extent permitted by law, waive all legal rights of action against and fully release Zero Gravity Pole, its Directors, instructors, employees, or related parties for the loss, damages, or injury howsoever arising out of or in relation to the participation by myself in the activities conducted or organised by Zero Gravity Pole including without limitation , liability for any injury related to pole dance or any other fitness session within the zero gravity studio. 

 

I understand that in the use of Zero Gravity Pole Dancing equipment, and in participating in zero gravity classes of any kind is at my own risk. This includes with without limitations:

 

• Use of all amenities and equipment in the facility and at off site locations and participation in any Activity, Classes, Program, or Instruction.

• The sudden and unforeseen malfunction of any of the equipment after being regularly checked and installed by professionals.

• Our instruction, Training, Supervision or recommendations.

 

We recommend that no alcohol be consumed by any participant prior to any class or activity within Zero Gravity Pole. Zero Gravity Pole will not be held responsible for any injury or death as a result of a participant being intoxicated. I understand and acknowledge the dangers associated with the consumption of alcohol or any mind altering drugs before and during the activity and I take full responsibility for any injury, Loss or damage associated with their consumption.  

 

CONDUCT 

 

I agree the follow the directions of the instructor and acknowledge that any misconduct or refusal by myself to follow direction of the instructor can result in the cancelation of my lesson and my immediate removal from the class or activity. I understand that nay such no compliance may result in injury or any kind whatsoever as a result of my failure to comply. 

 

HEALTH 

 

I acknowledge I have been advised to consult with my physician with respect to any past or present injury, Illness, Pregnancy or any other health related condition or any kind what soever that may affect my participation and ability  to participate in any program. I acknowledge the i have discussed the appropriateness of the PoleDancing Program and other fitness activities in connection with any illness or condition that i have or have had with my physician, and that I knowingly execute this release from liability. I acknowledge that Zero Gravity Pole does not recommend that pregnant women participate in our classes.    

 

PHOTO RELEASE

 

I, hereby grant permission to reproduce any portion of the photo images listed below that have been taken by Zero Gravity Pole Studio for the purpose of Studio Promotion publications which can include but is not limited to.

our social media presence. Furthermore, I grant creative permission to alter the

photograph(s). I do not grant permission to resale or use the photographs in a manner

that would exploit or cause malicious representation toward me or my company and

associates.
